Please note that the initial artwork you submit is considered a starting concept. Up to three design revisions are included at no additional cost. You will work directly with one of our designers to finalize your design and make any necessary adjustments.

Custom Uniforms Purchase Order Form
Please download our Custom Uniforms Purchase Order Form and begin completing it while a DeOro Sports representative reviews your submission. Completing the form immediately is not required, but it is greatly appreciated if you can submit it with your initial request. If not, the completed form must be submitted before your artwork is finalized and production begins.
The attached workbook has two tabs: Order Details and Lists.
Lead Times
Most orders ship within 2–3 weeks after artwork approval. Large orders — including orders with pants or 30+ units — typically ship within 3–4 weeks after artwork approval. You will receive an estimated delivery date at the start of your order. Expedited production options may be available for an additional cost.
No Art Fees or Set-up Fees
DeOro Sports does not charge artwork or setup fees to get started. Your order total will be based on our price list or team package pricing, plus applicable taxes and shipping.
Deposit & Payment
To begin the design process, we place a temporary authorization hold of $49.99 on your payment method. Your payment method will not be fully charged until the entire order is finalized and approved.
